The Situation:
Mr. AN (55 years old)
Mr. AN came to us with a history of retinal surgery in the right eye and bilateral PRP laser marks. His main concern was decreased vision in the right eye.
On examination:
IOP (eye pressure): Normal in both eyes
Fundus: Stable in both eyes
Optic nerve: No signs of glaucoma at the time of exam
We explained clearly to the patient and caregiver that:
His decreased vision in the right eye is due to damage from diabetic retinopathy and PRP treatment, not because of glasses.
At the time of examination, the patient did not request glasses; however, if he wishes, a separate refraction visit can always be arranged.
Glaucoma was not present during our checkup, and unnecessary anti-glaucoma treatment (AGT) was not required.
Why Vision Cannot Be Improved with Glasses in This Case
Retina problems usually have limited success even after surgery because the retina is a thin, delicate layer at the back of the eye. Once damaged
— often due to uncontrolled diabetes — its function cannot be fully restored with glasses or surgery.
In Mr. AN’s case, the optic disc looked pale after the PRP laser, which is a common after-effect and often mistaken for glaucoma.
This confusion sometimes leads to unnecessary labeling and treatment.
The real issue here is not glaucoma, but diabetic retinal damage.
The Most Important Step: Sugar Control
The patient’s HbA1c was 12 in past reports and the present reports show it is even above 12 , showing uncontrolled diabetes. No medicine for glaucoma
will protect his eyes if blood sugar remains this high.
✅ The most crucial step for him is to control his diabetes strictly.
✅ This will help protect the vision in his other (better) eye.
✅ By doing so, he can maintain independence in daily life.
